THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our Commitment to Your Privacy
The information you share with us about your health is personal, and we treat it that way. Elysium Wellness holds client health information to the privacy standards of the federal Health Insurance Portability and Accountability Act (HIPAA) and the California Confidentiality of Medical Information Act (CMIA). How We May Use and Share Your Health Information
To provide our services:
We use your health information to prepare for your sessions, review your labs, create your personalized protocol, and support you over time. With your direction, we may share information with your physician or other providers involved in your care.

To run our practice:
We may use your information to manage scheduling, maintain records, improve the quality of our services, and meet legal and professional obligations.
With our service partners:
We use trusted companies to help us operate, such as our secure client portal (Practice Better), laboratories that process tests you order, and our supplement dispensary (Fullscript). These partners may only use your information to perform services for us or for you, and they are required to protect it.
Appointment reminders and service updates: We may contact you by email, text, or through the client portal about appointments, results, and your care.
With people involved in your care. We may share information with a family member, friend, or caregiver you identify, or with a parent or guardian for a client under 18, as allowed by law. You may tell us at any time who should or should not receive information.

If you give us permission, you may take it back at any time in writing, except to the extent we have already acted on it.
Your Rights
